When emailing your Application, please attach the following required documents.
Please make sure all 4 corners of each document / ID are visible
Readable copies of all identifying labels.

  1. Drivers License or Passport – Front & Back – For each owner of 25% or more
  2. Proof of Address – Bank Statement or Utility Bill (Last 3 Months) – For each owner of 25% or more
  3. Corporation Formation Document – Articles of Incorporation
  4. Corporation Proof of Address – Bank Statement or Utility Bill (Last 3 Months)
